ALUMNI AWARD NOMINATION FORM

The Alumni Association recognizes alumni and one staff/faculty member who have achieved success in their chosen fields and those who have made significant contributions benefiting the University and/or the community-at-large. While the awards are presented at the annual awards banquet in February, nominations for the various awards described below may be made at any time during the year. Alumni submitting nominations should know that the individuals recommended for awards may be contacted for additional information. An awards committee will review all the nominations and choose the recipients.  Nominations are due by Monday, November 2, 2009.

Nominee resumes and at least two letter(s) of support must be submitted by email to alumni@nku.edu or faxed to 859-572-1950 for all nominations.
